AestheticHires.com combines BLS data with active aesthetic RN job listings and puts the national median for aesthetic RNs at $88,000/year.
ZipRecruiter shows $172,199. That figure is capturing NPs, practice owners and commission-heavy earners alongside entry-level staff. It’s not a reliable baseline for someone transitioning from hospital nursing.
How Commission Works in Aesthetic Nursing
This is a part of the salary article that most people neglect. It also is the most critical one.
Most aesthetic nurses don’t get a fixed income. They have an income from production. The reason is that you now have a knowledge of how that works.
The Three Pay Structures
- Pure salary. $55,000–$90,000 base, no commission. Widely used at high-volume med spas or for nurses that are new to the industry and developing their abilities. Predictable. Lower ceiling.
- Base + commission. This is the most popular model for established injectors. A base of $55,000–$72,000, plus 15–30% of the revenue the nurse personally generates. Here is where income can quite easily surpass the bottom line.

The Honest Assessment
The salary for aesthetic nursing is not as high as hospital nursing for the first year of practice. This is the true and the important thing to know before moving on.
You’re working on injection technique. You are starting from scratch to develop a patient. You are working on a commission or straight salary and the learning curve is significant.
Once they move up to year 3 and onwards (on the back of a commission structure and an increasing book), aesthetic nursing will more often than not have a higher earning potential than similar hospital nursing. The path goes up in a curve fashion, unlike hospital pay scales.
The ceiling: $200,000 to $300,000+ – no parallels in hospital staff nursing. There’s no ceiling of that sort.
One more thing: aesthetic nursing usually doesn’t involve working on nights or during weekends or holidays. Those hours are being worked by hospital nurses who are paid extras due to the differential. Aesthetic nurses aren’t. There’s a financial equivalent to that time difference, but you don’t need to work nights to obtain it.
